What should be entered in the Quarter Ending date in the Form 720?
June of the current year should be used for the Quarter Ending date as you are paying the fee in the second quarter of the current year.
What is the deadline to file?
The deadline to file is July 31 of the current year. If you are mailing the Form 720 and payment to the IRS, it must be postmarked no later than July 31.
Do I have to file for both my HRA and Self-Insured Health Plan?
If the HRA and self-insured health plan are integrated and have the same plan year (i.e., the health plan year and the HRA plan year both run from January 1 until December 31) you would only need to file for the self-insured health plan. If the plans are on separate plan years, then you would be required to file for both.
