If your benefits are administered through the BASIC, you can submit any form or file here through our secure website. Just fill out the requested fields and upload your file! If submitting data files, please note that the preferred file format for such files is Microsoft Excel (.xlsx or .csv).

Please Note: This upload portal only routes files to the BASIC offices in Califonia, and Akron, Ohio. If your benefits are administered out of a different BASIC location, you’ll need to contact that office for instructions on how to submit files.
